Fit (sick) note
A fit note, often called a sick note, is an official statement from a registered healthcare professional about a person's fitness for work.
If you have been off work for 7 days or less
You do not need a note from a GP.
If you are off work sick for 7 days or less, your employer should not ask for medical evidence.
You can tell your employer you've been ill by filling in a form yourself when you return to work. This is called 'self-certification'.
.If you have been off work for more than 7 days
You will need to request a fit note from the practice if you have been off work for more than 7 days in a row (including non-working days like weekends and bank holidays).
How to request a fit note
You can request a fit note by:

- Call the practice on +44 (0)1708 957100.
If you use our Online request form, you will need to:
- Select “I have an admin request”.
- Enter your patient details.
- Select "Fit (sick) note”.
- Enter the details about your sickness absence.
A GP will review your request. We may need to book a phone or in-person appointment to talk about your health before issuing a note.
