Urgent help at home (Community Treatment Team)

The Community Treatment Team (or CTT) is an NHS rapid response service that provides urgent medical care and therapy for adults at home.

The main aim of the service is to treat you safely in your own home when you have an acute (sudden) health problem, to help you avoid needing to go to a hospital A&E department.

Who this service is for

The CTT can help with urgent, physical health crises that are not life-threatening. This includes things like:

  • A fall where you are not seriously injured
  • A suspected infection (for example, a chest or urine infection)
  • A sudden worsening of a long-term condition, like COPD

The team includes doctors, nurses, therapists, and other healthcare professionals who can provide treatment and support at home.

How to get help from the team

You do not need a referral from a GP. Patients, family members, or carers can contact the team directly.

  • Hours: 8am to 10pm, 7 days a week

What to expect when you call

A nurse or therapist will ask questions about the situation to check if the service is suitable. If you are calling for someone else, they must be with you at the time of the call.

If the service is right for the situation, the team aims to visit within two hours.
